Jackson's Health: Family Health NP PLLC, is a private group of nurse practitioners located in Brighton, NY. We provide equitable and quality care to the Rochester community. We are a privately owned group that cares for patient's that are 17 years old and up. Our providers understand the needs of the Rochester community and are dedicated to optimize health and wellness. Our providers are Board Certified in Family Health and are dedicated to providing high quality, compassionate and equitable care for our patients and their families.
Missed Appointments
We set time aside for same day sick visits and appointments for acute problems. Missed and cancelled appointments impact the care of other patients. If you have to cancel or reschedule your visit, please do so 24-48 hours in advance. There is a $25 fee for missed appointments.
Insurance Policy
We aim to accept most major insurance companies within the next 3 months.
We aim to accept all major insurance companies within the next 3 months (Before July 15th, 2022). The following insurance plans are currently in process: MVP, MVP Option, Molina, Empire Plan, AETNA, Fidelis, Monroe Plan, and Cigna. If you do not see your insurance plan, please notify our team and we can possibly add your insurance carrier!
We are now accepting United Healthcare, Tricare, Emblem Health, Blue Cross Blue Shield, Blue Choice, Excellus, Medicaid and Medicare!
If you are a current patient, please use the patient portal to contact your provider. This will allow us to review your connected medical chart and provide personalized care.
