We aim to accept all major insurance companies within the next 3 months (Before July 15th, 2022). The following insurance plans are currently in process: MVP, MVP Option, Molina, Empire Plan, AETNA, Fidelis, Monroe Plan, and Cigna. If you do not see your insurance plan, please notify our team and we can possibly add your insurance carrier!
We are now accepting United Healthcare, Tricare, Emblem Health, Blue Cross Blue Shield, Blue Choice, Excellus, Medicaid and Medicare!

The Patient Services Department at Jackson's Health improves access to health care and assures that patients can access health care without insurance, and regardless of insurance status. Our BIPOC owned Sites offer access to quality health care for patients who don’t have insurance or don’t have enough insurance through a sliding fee discount program, with discounts available based on your current household size and income only. The program can cover you for up to 12 months—a new application is required yearly or when your household size and income changes, whichever is sooner.

Sliding Fee Scale Discount Program
The Sliding Fee Discount Program assures that patients without health insurance coverage have access to all primary care services. Eligibility for the sliding fee scale discount is based upon poverty guidelines established by the federal government and are determined by family size and income.

Privately Insured Patients
Patients with high-deductible health plans that require out-of-pocket expenses in excess of $2,500 annually will be extended the opportunity to apply for a sliding fee scale discount. Determination is based on documentation and verification requirements. If eligible, We will apply the sliding fee discount by charging a fee no greater than what would be charged to a patient that has no public or private insurance in a defined income category.

To apply for sliding fee services and alternate payment arrangements you will need to provide proof of household income. We will require a current valid photo I.D. and any one of the following items:

What is Direct Primary care?
We are keeping “Your care, Your way” by keeping our focus on you and your well-being, rather than the ever-increasing administrative tasks that detract from the time we can spend with you.
We believe this will allow us to care for you and your family in the way that you want. In place of billing insurance, Direct Primary Care will involve an affordable monthly membership fee that covers the services listed below. By eliminating the middleman, we can keep costs low and use our time to do what we are trained to do, help you maintain your health.

Membership benefits:
Easier access: generally same day or next day appointments will be available.
Cost efficient: Only one monthly payment for covered services.
Is Direct Primary Care Insurance?
No, we recommend that you keep your current insurance plan. If you can switch to a PPO plan, this often works better with Direct Primary Care.

Covered Services with Basic Membership
Same day or next business day office appointments Monday-Saturday, excluding holidays. Appointment type include wellness exams, acute and chronic disease management. Telemedicine offered 7 days per week.
Access to comprehensive primary care medicine via phone or telemedicine. Not all conditions or concerns can
be handled with these indirect methods and patient’s may be required to make an in-person appointment.

• Fingerstick glucose
• Stool test for blood

Excluded Services:
• Anything that is not listed above as a covered service (Always okay to call and ask for management advice or if it is covered)
• Sutures and staples are not covered
• Any healthcare services not performed on or within the premises of Jackson’s Health, including
emergency room visits, urgent care, hospital stays, specialty care, imaging, and labs, etc.
• Braces, crutches, and splints are not included but can be obtained in office

Membership fees:
• One adult individual: $118 per month
• One child (less than 18 years old): $65 per month
• (No fee per visit for up to 3 visits per month)
• $25 per visit thereafter
​
Reduced cost are offered below for:
• Individuals making less than $50,000/annually
• Families/Children with an annual household income less than $100,000/annually
​
Discounted fees:
• One adult individual: $90 per month
• One child (less than 18 years old): $50per month
• (No fee per visit for up to 3 visits per month)
• $15 per visit thereafter
